The Network provides comprehensive, coordinated care and services to kids with complex physical, developmental and communications needs who live in Simcoe County or York Region.

A Team Approach

Professionals from Network Partner organizations work together with your family to develop a coordinated Single Plan of Care that adapts to your child’s changing needs. Tools like a shared electronic record and common assessments allow professionals on your child’s team to share clinical information, coordinate services and monitor your child’s progress through all the stages of their development.

More Specialized Services Closer to Home

Network funding has also expanded the range of services provided by Network Partners so that more children with multiple special needs can receive the specialized services they need, closer to home.
