The Network removes barriers for families and expands the range of treatment and services that are needed in Simcoe and York to help kids with physical, developmental, and communication needs progress and participate fully in daily living, school and play.
The Network uses an entirely new team approach to meet the needs of children from birth to 19 with multiple disabilities. We link and coordinate existing services from over 40 healthcare, recreation, education, social and community organizations, so they can work together to develop a long-term plan of care that adapts to each child’s changing needs as they grow.
Over the next 3 years, all children with multiple disabilities in Simcoe and York will transition into the Network’s new single plan of care system, which includes developing an integrated care plan for each child, coordinating services and monitoring each child’s progress through the use of a shared electronic record.
Network partners will also continue to expand and add services that are critically needed in the community to help reduce waitlists and provide more services “closer to home”.
