How We Work Together
It’s all about focusing a team on the needs of your child and family.
Professionals from Network Partner organizations work together with your family, in Child and Family Teams, to set goals based on the family’s visions and develop a Single Plan of Care.
Your team is made up of all the professionals working with your child and family – at home, in school and throughout the community. The Network also works closely with organizations such as Sick Kids Hospital and Holland Bloorview Kids Rehabilitation Hospital to coordinate specialized services and care, where they are needed.
With your consent these professionals use new Single Plan of Care processes and tools, like a shared electronic record and common assessments, to share clinical information and work together as a team to plan treatment and supports, coordinate services and monitor your child’s progress. This reduces the need for you to tell your child’s story over and over again and allows your team to build on past progress and experience.
Parents are active members of their Child and Family Team and work closely with all team members to set goals and develop a Single Plan of Care. This care plan changes as your child develops and grows.
This is a new approach for providing comprehensive, coordinated care to kids with long term multiple special needs. Network Partners are working to integrate this team approach into their organizations and will gradually transition all children with multiple special needs to a Single Plan of Care.
