How We Work Together
Parents can access the network by:
- Calling 1-866-377-0286
- Completing a Contact Request Form and faxing it to 905-952-2412
- Being referred by a physician or agency
Services are provided through Child and Family Teams that are made up of professionals from different member agencies and organizations serving kids with special needs in Simcoe and York. The Network also works closely with organizations such as Sick Kids Hospital and Bloorview Kids Rehab to coordinate specialized services and care, where they are needed.
Upon contacting the Network, a navigator will take all the information needed to see if your child requires Network services, and will open an Electronic Client Record. If your child’s needs can be met by a single Network member, the navigator will connect you with the appropriate agency or organization.
Single Plan of Care
If several Network members are needed on your child/family team, the navigator will and arrange a comprehensive intake assessment, usually on the telephone. Team members involved with your child and family will work with you to develop a Single Plan of Care to suit your child’s specific needs and coordinate the services required as close to home as possible.
This plan will change as your child develops and reaches various transition phases (e.g. starting school, entering adolescence, etc.). - To be an active and valued member of your child’s team
- To provide ongoing input into defining and developing your visions for your child & family
- To be informed and asked for consent to share your personal information with the team
- To participate in the development of meaningful goals and activities for your child and family
- To participate in your child’s plan and treatment
