Minister Hands-On at New Resource Centre
The Children's Treatment Network is a new service delivery model for kids with multiple disabilities in Simcoe and York. It links over 30 healthcare, education, recreation, social and community services organizations so they can take a team approach to each child's care. For the first time, parents have one point of contact for a single plan of care for kids from birth to age 19. The Network coordinates services needed and monitors each child's progress through all the phases of their development. The Network is also expanding and adding services that are critically needed in the community to bring more services closer to home and help reduce waitlists. While touring the new Children's Treatment Network Resource Centre in Barrie on November 30, Minister of Children and Youth Services, Mary Anne Chambers added her handprints to a special commemorative canvas created by local children with multiple disabilities. The Resource Centre in Barrie provides local families with access to range of specialized services and professionals, all at one location, right at their doorstep. Another Resource Centre located in York Region will be opening in spring, 2007. Network services are free and are funded by the Ministry of Children and Youth Services. Local Team Facilitators On Board.

The Network's new Local Team Facilitators have hit the ground running! Nine of the 10 professionals leading Local Teams in Simcoe and York are hard at work learning new cross-discipline systems and processes that will support Local Team members from over 30 organizations to develop a single plan of care for each child and implement common clinical standards.

Electronic Client Record Launched!

On November 27, therapists, service coordinators, service navigators and clinicians from 20 Network Partner organizations were the first to receive training on a pilot version of the new Electronic Record. Many thanks to CCAC York Region for providing an excellent venue for training on this critical new tool.
The Electronic Client Record supports the development of a single plan of care and will be used by all Partners across the Network. It allows multi-disciplinary teams across multiple organizations to conduct a team assessment, develop an integrated plan of care, share confidential information, view clinical notes and monitor each child and family's progress. This clinical software application - singleplanofcare.com - will be further developed throughout 2007, and training will be rolled-out regularly to more front line staff in Partner organizations.
Training Kicks Off for Front Line Staff to pilot Integrated Care Plans

Currently, integrated care plans are under development for 25 high needs children. It is expected that a total of 50 children will be incorporated into this pilot program by the end of January. To initiate the pilot, the first in a series of training sessions for Network Local Team professionals was held on November 30 at Simcoe Muskoka Catholic District School Board.
Over 40 service coordinators, therapists, service navigators, educators and other professionals from 9 Network partner organizations working on specific child and family teams took part in the interactive, experiential session. Front line staff training includes information on systems, processes and protocols that support single plan of care development and integrated delivery of services from across all 34 Network partner organizations. Training sessions for Local Team professionals will continue throughout 2007.
New Reference Guide for Professional Now Online
A comprehensive kit of information for professionals is now available online at http://www.ctn-simcoeyork.ca/resources/referenceguide/index.php Reference material includes details on the Network's integrated, trans-disciplinary model as well as details on systems, tools, process and protocols.
