Referral Information for Physicians- To refer a child and family for Network Services please complete the downloadable CTN Physician Referral Form and fax to 905 952-2412. 

 

Referral Information for Service Providers- To refer  a child and family for Network Services please complete the Request for Local Team Services Form.  For Specialty Services please complete the Request for Specialty Services Form as appropriate and attach relevant documentation with the form.  Information must be complete in order for the referral to be processed.  Fax the completed forms to

Referral Information for Families- A family may self refer their child without a physician referral by calling CTN Access at 1-866 377 0286
