Physicians - Please complete the downloadable Physician Referral Form and fax to 705-792-2775
Service Providers - Please complete the Request for Local Team Services Form. For Specialty Services please complete the Request for Specialty Services Form and attach relevant documentation. Information must be complete in order for the referral to be processed. Fax completed forms to our new fax number 705 792-2775
